PROGRAM – CPSAI
POSITION – ASSESSMENT COUNSELOR
We are currently seeking an Assessment Counselor to work in our CPSAI Program located in Hunterdon County, Flemington, NJ. This is a full time position with a generous benefits package.
Qualifications: Masters Degree in Social Work, Psychology or related field or Bachelors Degree in Social Work, Psychology, or related field and experience working with the addicted population or NJ State License/Certificate in Substance Abuse (LCADC/CADC) and Valid driver’s license
minimum of a CADC or bachelors degree.
Specific Responsibilities/Functions/Competencies:
- Thoroughly completes all assigned tasks within prescribed time frames and demonstrates reliability with consistent follow up;
- Maintains current, respective credentials and/or licensure status;
- Written communication is legible, concise and adequate;
- Attends and participates in external training as relevant to improving job skills and/or maintaining CADC/LCADC status;
- Arrives at work punctually;
- Complies with work schedule as negotiated with supervisor;
- Demonstrates collegial attitude and cooperative behavior with co-workers within department/program;
- Participates in Clinical Rounds and Consortium meetings by sharing information, proposing ideas, and providing feedback to enhance service delivery;
- Seeks appropriate and timely consultation from supervisor, and takes initiative to report relevant issues to supervisor;
- Performs job responsibilities and duties with a minimum of Supervisory prompting;
- Effectively provides assessments and recommendations for DCP&P referred clients;
- Effectively utilizes ASAM PPC-II criteria and the LOCI-2R and refers clients to the most appropriate level of care;
- Plans and coordinates all substance abuse services and monitors compliance;
- Presents cases in a coherent manner that demonstrates appropriate clinical judgment and knowledge;
- Completes assessments utilizing the NJSAMS, DSM IV Criteria, ASAM PPC-II Criteria, LOCI-2R, drug screen results and collateral information;
- Completes assessments with a high degree of attention to detail and accuracy, utilizing clinical knowledge as well as collateral information;
- Completes case closure forms;
- Keeps DCP&P informed of client status according to CPSAI guidelines and providing DCP&P with appropriate notifications that require their signature;
- Provides DCP&P with treatment recommendations (which include Diagnostic Impressions/Recommendation Page, Pre Placement and Narrative within 5 business days of assessment and obtains DCP&P signature;
- Takes responsibility as case manager for the completeness of the entire clinical record for all cases assigned;
- Completes all reports in a timely manner and obtains DCP&P signature on all necessary forms. Provides copies of these forms to DCP&P;
- Completes Progress Notes (including Initial Assessment Note) and all communication with client and case conferences with DCP&P;
- Completes Consortium Forms accurately and file in the charts when applicable ;
- If a client missed an appointment an Incomplete Assessment Notification is completed by CPSAI and signed by DCP&P;
- Submits monthly tracking form to Administrative Assistant within the first week of the month;
- Upon receiving referral, conducts assessment in the time indicated according to the level of priority;
- Outreach efforts are made to clients; phone calls, home visits, etc;
- Establishes and maintains strong ongoing relationships with area addiction treatment agencies;
- Monitors, collects and handles urine specimens using chain of custody procedure. Completes related documentation, and observes throughout the procedure;
- Practices universal precautions;
- Enters referrals into NJSAMS upon receipt and update accordingly to missed appointments;
- Update the Outcomes Page in NJSAMS as you monitor their progress upon entering treatment;
- Update NJSAMS if changes in level of care should occur;
- Discharge in NJSAMS;
- Ensure tracking reports match NJSAMS information.

Additional Duties: Attends monthly clinical rounds; attends mandatory orientation and trainings; demonstrates ability to perform basic computer functions as required by job function; performs job responsibility with a minimum of supervision from Supervisor; follows policies and procedures outlined in Employee Handbook; attends at least two agency held quarterly meetings per year; demonstrates familiarity with organization and program specific policies; all other duties as assigned.
